Many times we notice that certain point on our foot hurts a lot when walking, running or simply touching foot to the ground and when we start inspecting the sole of foot we notice a thick, hardened circular lesion that causes the pain. That is called as the corn. Mostly corns develop on the feet and toes or hands and fingers.

What is a corn?

A corn is a skin lesion of thick, hardened dead skin caused due to pressure, usually over the toe knuckles. It can form a hard “seed” or “corn” in the centre of surrounding callous (hard skin)..

There are 2 types of corn’s soft corns and hard corns. Soft corns usually occur between toes and they stay soft due to moisture in between toes. Hard corns are more common on top of toes and on the side of the small toe

Clinical Presentation of corns

  • A thick, rough area of skin
  • A hardened, raised bump
  • Tenderness or pain under your skin
  • Flaky, dry or waxy skin
  • Present on top and sides of toes

Why do we seem to only have corns on the feet?

Because our feet are stuck inside hard shoes, any regular pressure against a prominent toe joint can cause a callous or corn or multiple corns. The whole of the body pressure is taken by feet and when this is combined with shoe pressure, the corn or corns are formed.

Why won’t it go?

Corns can be very stubborn and not respond to padding, chemical applications such as corn plasters and chiropody treatment. Underneath the corn there is a lump of inflamed tissue which forms in response to soft tissue inflammation, called a bursa. This then further increases the pressure underneath the corn, making it more sore and prominent.

The procedure of corn removal is called a skin plasty, which removes the corn completely, and the bursa underneath. In most cases we will recommend either a small joint fusion or an arthroplasty.

First the area is made numb using local anesthetic. Then a circular incision is made around the area of corn and slowly and carefully the corn is then excised or cut out using a fine scalpel. With an electrocautery, hemostasis is achieved and stitches may be necessary. A small bandage may be applied on top. Entire procedure can be completed in about 30 minutes and it can be performed in doctor’s office. You should be completely recovered in 3-7 days as it is a minor surgery. It is recommended  to keep the activities mild until you are fully recovered. There is very mild pain which can be controlled with painkillers.

Guided laser is used in this procedure in order to burn off the corn. The recovery time depends on the location of the corn. Preventive actions to prevent corns from returning.

  • Using properly fitted shoes with good padding and with a wide toe box. If the shoe box is tight get it stretched from a cobbler.
  • Using thick socks to absorb pressure on the foot can be useful. Use socks that are loose fitting and do not make your shoes too tight.
  • Adjust your walking style. An improper gait, such as walking on the sides of your feet, can produce calluses and corns.
  • Protect your skin.
